Out of Network Options

As a courtesy, we will submit a claim to your health insurance plan for which we are not preferred providers with. We will verify coverage, get information and provide an explanation of physical therapy benefits for you. We do not accept a non-participating fee. The estimated co-insurance will be a balance due after the insurance company processes their portion of coverage. Any balance due or denied services will be the responsibility of the individual.
