If you are covered by any of the above listed plans, please have all the required information at the time of your visit:

1. Your current insurance card.
2. Prescription from your physician for physical therapy services.
3. Written referral from your primary care physician, if required by your insurance.

IF YOU DO NOT PROVIDE THIS INFORMATION ON YOUR FIRST INITIAL VISIT OR IF THE INFORMATION IS NOT SENT PRIOR TO YOUR VISIT, YOU WILL BE EXPECTED TO PAY FOR THE INITIAL VISIT AND ANY SUBSEQUENT VISITS AT THE TIME OF SERVICE.

As a courtesy, we will submit your claim to insurance plans that we do not participate with. We will provide you with their benefits explanation. This is not a guarantee of payment from your insurance plan and your portion of the co-insurance will be expected at the time of service. WE DO NOT PARTICIPATE WITH HMO PLANS.
