General Insurance Information

Our administrative staff will be happy to address any questions or financial concerns that my rise during the course of your treatment.

Any co-pays, co-insurance, deductibles and self-pay payments are expected at the time of service in the form of Checks, Cash, MasterCard, Visa, Discover, or American Express.

It is the patient's responsibility to know the requirements of their insurance carrier for physical therapy services. The prescription and/or referral for physical therapy services must be current and within 30 days of beginning physical therapy. It is also the responsibility of the patient to monitor the number of authorized visits for physical therapy.

Individuals are financially responsible for rehabilitation supplies not covered by their insurance company. Such as, anti-inflammatory medications, braces, taping or customized orthotics.
